Our Mission
The mission of Caregivers Health Network,
Inc. is to provide skilled, compassionate nursing and rehabilitation
services to those we serve. We will make every effort to learn
more, to care more, and to listen more to our clients, families
and staff, constantly striving to be worthy of our name "Caregivers".
Our Staff
Our dedicated staff of professionals, including RN’s, LPN’s,
Physical, Speech & Occupational Therapists, Medical Social
Workers and Home Health Aides combine years of experience and
skill with compassion to bring our clients the services and support
that they need to remain at home.
Our Philosophy
It is our philosophy that by providing and coordinating total
home health
services, we are able to help our clients maximize their independence
while receiving care in a comforting environment, surrounded by
family and friends. Family members and friends have the peace
of mind knowing that we have addressed all of our clients’
needs through quality care in order to provide a safe, secure
environment at home.
Continuity of Care
Caregivers Health Network, Inc. works with you and your Physician
to provide the latest in comprehensive home care services through
our visit and extended hours programs. We are available and willing
to help 24 hours a day, 7 days a week.
Our services are offered under the supervision of our Medical
Director and Administrative Staff. We work directly with your
Physician to develop an individual plan of care to meet your specific
needs.
Our Nursing and Therapy Team work together to establish therapeutic
goals to achieve maximum rehabilitation for each client. This
plan of care is implemented by the team, and reviewed and assessed
routinely to ensure that all needs are met.
We work with our colleagues in the health care community to provide
our clients with all services, support measures and equipment
available to make each home care experience a success.
